Pour l’exécutif actuel, il n’y a pas de mauvaises économies budgétaires. Quand bien même ces économies concerneraient un service public déjà à l’os. Et quand bien même ce service public revêtirait une importance capitale pour la population. On parle ici de l’hôpital public. Selon Les Échos du 12 août, « l’inefficacité des hôpitaux » serait ainsi dans le viseur du gouvernement qui compte couper à la hache dans le budget de l’année 2026.
Rappelons que François Bayrou a fixé à 44 milliards d’euros l’objectif d’économies pour l’année prochaine, afin de ramener le déficit de 5,4 % à 4,6 % du PIB en 2026. S’il est encore aux affaires, il compte faire plus encore les années suivantes pour passer en dessous de la barre des 3 % en 2029, comme promis à Bruxelles. Un coup de frein budgétaire qui risque de ralentir encore davantage la croissance économique déjà atone – elle devrait s’établir à 0,6 % en 2025, selon l’Insee.
Hélas, au lieu d’aller chercher l’argent là où il est, que ce soit en questionnant les 211 milliards d’euros par an d’aides aux entreprises, ou en rabotant les dispositifs permettant l’optimisation fiscale des grands groupes et des plus riches, le gouvernement préfère pousser les hôpitaux à renforcer « leurs performances ».
« Une plus grande efficacité sera demandée à l’hôpital », avait annoncé le 15 juillet François Bayrou. Il a aussi dit qu’il demanderait à l’hôpital public de « faire des économies en optimisant par exemple les achats de médicaments ou en s’appuyant sur la médecine ambulatoire ».Le même exécutif a chargé l’hiver dernier l’Inspection générale des finances et l’Inspection générale des affaires sociales (IGF et Igas) de trouver des pistes pour « renforcer la maîtrise de la masse salariale » et d’étudier « l’opportunité d’une évolution de la carte hospitalière ». Les conclusions des deux services sont attendues à l’automne pour les débats budgétaires.
Désengagement de la Sécurité sociale
Face à cette petite musique qui monte d’une cure d’austérité à venir pour l’hôpital public, le président de la Fédération hospitalière de France, Arnaud Robinet, ne décolère pas. « J’ai entendu certains dire que l’hôpital public ne serait pas efficient, cela me met hors de moi », a-t-il aussi déclaré aux Échos.
Certes, quand l’on se plonge dans le dernier rapport datant du 24 juillet de la Direction de la recherche, des études, de l’évaluation et des statistiques (Drees) sur les comptes des hôpitaux publics, on comprend que leurs finances sont loin d’être d’équerre. Leur déficit est en nette hausse et a atteint près de 3 milliards d’euros en 2024.16 juillet 2025
Mais ceci n’est en rien dû à des hôpitaux qui seraient rendus obèses par leur train de vie somptuaire. Le déficit des hôpitaux se creuse en fait à cause, d’une part, de la baisse relative des remboursements par la Sécurité sociale de la croissance de leur activité, largement portée par l’ambulatoire.
Et d’autre part, la Drees a constaté une nette hausse de leur masse salariale « en lien avec les mesures nationales de revalorisations des rémunérations dans la fonction publique hospitalière ».
Or, il est difficile d’avancer aujourd’hui que le personnel hospitalier roule sur l’or. Certes les fonctions hospitalières, notamment le personnel soignant, sont davantage soutenues financièrement depuis la crise du covid-19, mais les revalorisations salariales les concernant avaient été quasi inexistantes durant la décennie précédente.
Du reste, depuis le choc de la crise sanitaire, la nécessité d’allouer davantage de moyens à l’hôpital public était communément admise pour assurer un niveau de soin de qualité pour la population. Même Emmanuel Macron, en mai 2020, avait concédé – fait rarissime – un mea culpa au sujet du sous-investissement dans l’hôpital public dont l’État avait été, selon lui, coupable depuis son arrivée au pouvoir.
« On a sans doute fait une erreur dans la stratégie annoncée », avait-il accordé. Son entourage reconnaissant que les deux plans de réforme de l’hôpital public lancés en 2017 et en 2019 « étaient sous-dimensionnés ».
Mais cinq ans après le covid-19, il n’est plus question pour l’exécutif de faire des sentiments vis-à-vis des soignant·es. La priorité « vitale » pour la France est désormais de réduire le déficit et la dette par le biais de la baisse des dépenses publiques, martèle François Bayrou.
Une véritable saignée est donc attendue. Et c’est d’autant plus compliqué à accepter pour la population que c’est ce même exécutif qui a creusé le trou dans les comptes publics. Près de 70 milliards d’euros se sont en effet évaporés des comptes publics en 2023 et en 2024 à cause d’une erreur d’appréciation du gouvernement et de ses services concernant le niveau des recettes fiscales.
Des réformes en masse
Une erreur que le gouvernement Bayrou fait désormais payer aux services publics et au modèle social. Car en parallèle, il ne compte surtout pas déroger à la politique de l’offre mise en place depuis 2017, faite de baisses d’impôts sur le capital, d’allègements de cotisations sociales pour les entreprises, et de mesures visant à flexibiliser le marché du travail.
Tous ces dispositifs ont eu un coût pour les finances publiques : plus de 50 milliards d’euros de baisses d’impôts ont été votées ; les allègements de cotisations étaient de 25 milliards d’euros plus élevés en 2024 qu’en 2017 ; et la forte hausse du nombre d’autoentrepreneurs – 800 000 de plus qu’en 2017 – sont autant d’emplois qui cotisent moins qu’un salarié à la Sécurité sociale.
Hélas, ces baisses d’impôts n’ont pas généré le ruissellement promis en termes de croissance économique et de recettes fiscales pour l’État.
Pour financer ce manque à gagner et hypothétiquement tenter de rentrer dans les clous de Bruxelles comme il s’y est engagé, l’exécutif a alors choisi de taper fort dans les seuls postes de dépenses qui peuvent lui rapporter des milliards en très peu de temps : ceux de la protection sociale.15 juillet 2025
Les retraites d’abord, avec la réforme de 2023 qui repousse l’âge légal de départ de 62 à 64 ans. L’assurance chômage, ensuite, qui a subi pas moins de quatre réformes depuis l’arrivée d’Emmanuel Macron au pouvoir, et qui pourrait bien en connaître une cinquième.
François Bayrou a en effet envoyé le 8 août aux partenaires sociaux une feuille de route pour une nouvelle réforme qui permettrait de générer 4 milliards d’euros d’économies supplémentaires par an à partir de 2030.
Reste, enfin, la santé, qui est la nouvelle martingale à économies budgétaires du gouvernement. Contrôles accentués des arrêts maladie, baisses de la prise en charge des affections de longue durée (ALD), déremboursement de médicaments... François Bayrou compte désormais gratter tout ce qu’il peut au sein de ce poste de dépenses pour trouver des milliards.
Il compte même aller jusqu’à reprendre les cannes anglaises et les fauteuils roulants dans les logements des personnes décédées, c’est dire ! « J’ai toujours été frappé que les dispositifs » comme « les fauteuils pour les personnes infirmes gravement malades en fin de vie, les cannes anglaises » ne soient pas « réutilisés après que la personne n’en a plus besoin, par exemple après qu’elle a disparu », a-t-il déclaré le 15 juillet.
Peu surprenant dès lors, que l’hôpital, le premier poste de dépenses de santé en France – 122 milliards d’euros sur 249 milliards d’euros au total – soit désormais en plein dans le viseur de l’exécutif. Après avoir connu un sous-investissement chronique au regard de la hausse des besoins en soins ces dernières années, l’hôpital public pourrait donc subir en 2026 ses premières coupes budgétaires drastiques depuis la crise du covid-19.

CrowdBucks is a new effort developed by Charles Iliya Krempeaux, better known by his online moniker, Reiver. It builds on some of the ideas previous implementations have tried, and aims to make the process as smooth and simple as possible.
“The long-term vision that CrowdBucks is a part of is to create a payments layer for the Fediverse,” Reiver explains, “obviously, it’s not the only part, there will be other projects later.”Source: CrowdBucks
Instead of forcing users to create yet another account, CrowdBucks does something really smart: you can just sign in with an existing Fediverse account.
At the moment, sign-in is limited to just Mastodon, but the plan is to gradually support a number of different platforms. Since a lot of Fediverse software implements part of the Mastodon API, I attempted to log in with both Akkoma and WordPress, but neither one seems to work yet. We opted to use a tried-and-true community instance.
After doing the Authorization dance, CrowdBucks directs users to a simple dashboard, where they are prompted to do basic setup for their page. Fediverse integration automatically pulls in profile details, including the username, avatar, header, and handle, although most of the public-facing details can be customized.
The first thing to do with your account is to set fundraising goals and donation tiers. The flow feels reminiscent of something like Kickstarter or Patreon, where rewards can be spelled out as something symbolic, something digital, or even something tangible.You can view our demo account here. Support tiers can be set up with any monthly denomination, and these get prominently displayed on your CrowdBucks page. Donors can use their CrowdBucks accounts to find a page, select a tier, and support creators and projects easily.
When a person pledges towards a Tier, they’re automatically taken to a checkout page. For the time being, the only supported Payment Processor is Stripe. Reiver has explained that this is because Stripe was easiest to implement, but the team intends to also add support for PayPal and other providers, as well as support for standards such as Web Monetization and OpenPayments.
CrowdBucks was initially revealed in a brief demo at FediCon a few weeks ago, which was recorded and added alongside the FediCon Talks on PeerTube. It’s a useful insight into where Reiver is coming from, what’s being built, and ideas of what CrowdBucks could be used for.

“Anyone will be able to set up their own CrowdBucks server,” Reiver explains, “just like anyone can set up their own Mastodon server.”
The CrowdBucks project itself is licensed under the GNU AGPL, with source code readily available. The CrowdBucks.fund site is simply operated as a flagship instance, but the goal is to allow anyone to host their own version as part of their operational infrastructure.
“We want CrowdBucks to help pay server bills, to support developers building Fedi software, and to fund creators on the Fediverse. The whole thing is designed to be native to the Fediverse.”
While the project itself is still fairly young, the team is actively thinking about how to improve. One area CrowdBucks is already exploring involves the ability for the app to post to the Fediverse on the behalf of fundraisers, for example, to give credit to supporters. Another possibility might involve collaborating with Emissary to standardize pieces involving payments and private access.
This is an exciting endeavor, and might be one of the most polished attempts yet to make payments possible on the Social Web. Hopefully, existing projects will get involved, and hash out the details on how to make this as open and interoperable as possible.
