The current diagnostic criteria (CDC) does require PEM:

It's possible there are atypical cases (or subgroups of ME/CFS ? ) without PEM. I have no idea. But that's not the current definition of ME/CFS.

My understanding is that if a patient meets the ME/CFS diagnostic criteria then they have ME/CFS. The cause is not specified in diagnostic criteria.
Most cases of ME/CFS, but not all, are triggered by a viral infection. SARS‑CoV‑2 is just one possible virus. Flu (H1N1) and other viruses have also been found to trigger ME/CFS.
